Uniform Data Management System
 

As part of its contract with VCU, a uniform data management system was developed that allowed BPAO contractors to submit, revise, and aggregate information on their clientele via web-based forms. The National BPAO Data System consists of the following forms:

  • Project Site Form -Each BPAO project must be registered into the system and receive a unique identifier. The form requests contact information and the date on which the project initiated services.
  • Benefit Specialist Form -Each Benefit Specialist within a project must also register and receive a unique identifying number. The form also requests contact information and the date that the Benefit Specialist initiated services.
  • BPAO Beneficiarv/Recipient Form -This form is completed following contact with a BPAO consumer. It requests identifying information (name, Social Security number, address), key demographic information (age, sex, disability), current benefits received, current employment status, reasons why the individual sought BPAO services, the types of services delivered, work incentives recommended, the amount of time required for the contact, and whether or not an employment change is anticipated on the part of the consumer as a result of the services and assistance provided.
